Statutory Forms and Notifications

Services are required to notify the MHC of various practices and events. These include quality and safety notifications and statutory forms. 

Quality and Safety Notifications

From 1 January 2023, all Quality and Safety Notifications (QSN) must be made through CIS. This means that the MHC will require notifications for seclusion, physical restraint and mechanical restraint to be submitted via CIS and that paper-based forms used previously to submit any notification relating to Seclusion will not be accepted from this date onwards.
